The Howard University Middle School of Mathematics and Science – (MS)participates in the Washington, DC citywide public school application and common lottery through My School DC. Lottery results for school year 2020-21 are now available in your My School DC family account. If you missed the lottery application deadlines or want to add our school to your child’s existing application, you can add your child to our schools’ waitlist today by submitting a post-lottery application. Enrollment Requirements

Enrollment Form

Families who have been matched with the Howard University Middle School of Mathematics and Science will receive an email with a link to our enrollment form. Our enrollment form is completely online and has simplified the entire process. To begin your child’s enrollment, please complete the online enrollment form and submit select supporting documents (student birth certificate, parent identification, etc) at your earliest convenience.

Additional Required Documents

Once you submit the enrollment form, you will receive an email within two business days with the next steps and directions on submitting the additional required documents. Parents will have the ability to sign and submit all required forms and supporting documents electronically.

SY 20-21 My School DC Seat Acceptance Form
Parents must complete this form to confirm your child accepts a seat in a My School DC school and submit it with other enrollment requirements to the school.

Home Language Survey
The Home Language Survey is used to determine if the student is eligible to take an English language proficiency screener. The screener score determines if the student is identified as an English learner or not an English learner. Students who are identified as English learners have the right to participate in the English language instructional program at school.

DC Residency Verification
All students enrolled in public schools in the District of Columbia (DCPS and charter) must provide proof of residency in the District. Residency verification usually takes place upon enrollment at the school.

Please be advised that OSSE will now require that students and families provide Universal Health Certificates and Oral Health Assessments at the start of the 2020-21 school year. However, all students must be up to date on their immunizations in order to attend school.

Health Forms
The required health forms consist of: District of Columbia Universal Health Certificate and District of Columbia Oral Health Assessment Form. Given the changes to the enrollment process, medical and dental forms are no longer required at the time of enrollment. OSSE will now require that students and families provide Universal Health Certificates and Oral Health Assessments at the start of the 2020-21 school year. However, all students must be up to date on their immunizations in order to attend school. Parents who have access to up-to-date medical and dental forms are encouraged to upload them during the enrollment process, or as soon as they become available to you.
